Reflect on the reasons that you took time away from Notre Dame. Explain how you will manage a similar set of circumstances should they arise once you return to Notre Dame.
Describe two situations, one personal and one academic, when you sought help and utilized positive coping skills to deal with a stressful situation while away.
Share the changes you have made that have prepared you to return to your studies.
Describe what you have learned about yourself during your time away.
Present a plan with specific goals designed to sustain a positive return to Notre Dame. This plan should identify at least three resources on or off campus that you can access for support.
